North Tyneside Council's Adult Social Care is seeking two dedicated and experienced Adult Social Workers to join our dynamic Community Wellbeing Teams. We're looking for individuals committed to supporting adults in need, promoting their independence and well-being.

... - One Adult Social Worker to carry a caseload: This role will be predominantly home-based with a blended working model, 3 days in the office and 2 days working from home.
- One Adult Social Worker to work within the duty team: This role will be office-based for the first 5 days, allowing you to become familiar with processes and systems. After that, you will work 5 days in the office.
About the Role and Responsibilities:
Community Wellbeing Team:
- Assessment and Support Planning: Conduct comprehensive assessments of adults referred to the Community Wellbeing Teams, identifying their needs, strengths, and risks. Develop and implement individualised care plans, including interventions and support strategies to promote independence and well-being.
- Delivering Effective Interventions: Work collaboratively with individuals, families, and other professionals, including health services, housing providers, and the police, to deliver timely and effective support.
- Hospital Discharge Work: Play an active role in the discharge process, supporting individuals to transition from hospital into the community, ensuring continuity of care and positive outcomes.
- Planning Meetings and Reviews: Participate in planning meetings and reviews to ensure that care plans meet individuals' changing needs and are continually evaluated.
- Providing Information and Advice: Provide information, advice, and guidance to individuals and families on a range of social care services and benefits available.
- Advocating for Individuals: Advocate for the needs of individuals, ensuring their rights and well-being are protected.
- Maintaining Accurate Records: Maintain accurate records and documentation, ensuring adherence to relevant legislation and guidelines.
Duty Team:
- First Point of Contact: Receive and assess referrals for adults requiring urgent social care support, acting as the first point of contact for individuals and their families.
- Prioritizing and Managing Urgent Cases: Prioritize and manage urgent cases, ensuring timely responses and appropriate interventions.
- Supporting Colleagues: Provide support and guidance to colleagues within the team, ensuring effective communication and knowledge sharing.
- Contribute to Service Development: Participate in quality improvement initiatives and contribute to the development of efficient and effective service delivery within the Duty Team.
Key Requirements:
- Qualified Social Worker (DipSW, CQSW, BA Hons in Social Work, or equivalent).
- Social Care England registration.
- Updated DBS check.
- Experience working within Adult Social Care, particularly in a community-based setting (for Caseload role) or in a Duty team environment (for Duty role).
- Excellent communication and interpersonal skills.
- Strong analytical and problem-solving abilities.
- Ability to work effectively under pressure and manage competing demands.
