Responsibilities:
- Provide support and guidance to adults and older people living in the community, helping them achieve their goals and maintain their independence.
- Manage a diverse caseload with autonomy, allowing for a direct impact on the lives of those served.
- Conduct thorough assessments and reviews to determine the needs of individuals, creating effective care plans that meet their unique requirements.
- Develop comprehensive care plans, ensuring individuals receive the appropriate support and resources to thrive.
- Prepare clear and detailed reports to document progress and share vital information with relevant parties.
- Work directly with individuals requiring support under the Adults with Incapacities (AWI) framework.
- Advocate for individuals who may be at risk of harm or abuse under the Adult Protection (AP) guidelines.
Requirements:
- Member of the PVG scheme
- Degree or equivalent in Social Work
- Registered with the Scottish Social Services Council (SSSC)
- Council Officer trained
- Strong knowledge and experience of the relevant legislation
