🔍 About the Role:

You'll be working with adults with learning disabilities and/or physical disabilities to assess needs, promote independence, and ensure access to the right support and services. The role requires a strengths-based, person-centred approach in line with the Care Act 2014 and relevant legislation.

✅ Key Responsibilities:

• Complete Care Act assessments, support plans, and reviews

• Coordinate care packages and liaise with providers and health professionals

• Carry out Mental Capacity Assessments and manage safeguarding concerns

• Promote independence and community-based solutions

• Maintain accurate, timely records and reports in line with statutory duties

🎓 Requirements:

• Qualified Social Worker with Social Work England registration

• Experience in adult social care, ideally in a disabilities team

• Strong working knowledge of the Care Act, MCA, and safeguarding procedures

• Excellent assessment, planning, and communication skills

• Ability to work independently and within a multidisciplinary team
