The Role

You will be working with adults who have complex needs, including older adults, those with physical disabilities, learning disabilities, or mental health issues. Your role will be to assess, plan, and coordinate care and support services, ensuring the safety and well-being of individuals in the community.
Key Responsibilities
- Carry out comprehensive needs assessments and develop personalised care and support plans
- Work collaboratively with service users, their families, and other professionals to provide person-centred solutions
- Undertake safeguarding investigations, ensuring that vulnerable adults are protected
- Provide advice and advocacy to individuals, supporting them to access services and achieve greater independence
- Manage a varied caseload effectively, ensuring timely interventions and reviews
- Maintain accurate case records in line with legislative and organisational requirements
- Work in partnership with health services, voluntary organisations, and other local authority departments to ensure a multi-agency approach to care
- Contribute to service development by sharing best practices and participating in team meetings
About You
- Qualified Social Worker with a valid registration with Social Work England
- Proven experience in working with adults in a social care setting
- Knowledge of relevant legislation, including the Care Act 2014, Mental Capacity Act, and Safeguarding Adults frameworks
- Excellent assessment, care planning, and case management skills
- Strong communication and interpersonal skills, with the ability to work collaboratively with service users, families, and professionals
- A proactive, solution-focused approach, with a passion for supporting individuals to achieve their full potential
- Ability to work independently and manage a busy caseload effectively
