💼 About the Role:
As a Social Worker within the Learning Disabilities Team, you'll be working closely with adults and their families to promote independence, safeguard wellbeing, and support person-centred care planning. You'll play a vital role in assessments, reviews, and multi-agency working, ensuring that support packages are tailored and empowering.
✅ Key Responsibilities:
Conducting Care Act assessments and reviews
Creating and implementing person-centred care plans
Working in partnership with health professionals, families, and support providers
Safeguarding vulnerable adults through robust risk assessments and protection plans
Maintaining accurate case records in line with statutory responsibilities
🧠Ideal Candidate:
Qualified Social Worker (Degree/DipSW/CQSW)
Current registration with Social Work England
Experience working with adults with learning disabilities
Strong understanding of the Care Act, MCA, DoLS, and safeguarding procedures
Able to work autonomously and as part of a collaborative team
