🧠 About the Role:

As part of the Mental Health team, you'll be working with adults living with complex mental health needs, supporting them to live as independently and safely as possible. You'll conduct statutory assessments, coordinate care plans, and collaborate with health and social care partners to ensure positive outcomes.

✅ What You'll Be Doing:

• Carrying out Care Act assessments and mental health reviews

• Developing and managing person-centred support plans

• Undertaking safeguarding work and risk assessments

• Applying the Mental Capacity Act and Mental Health Act where required

• Working closely with service users, families, and multi-agency teams

🎯 What We're Looking For:

• Qualified Social Worker with current Social Work England registration

• Experience working in adult mental health services

• Sound understanding of relevant legislation (Care Act, MCA, MHA)

• Strong interpersonal and case management skills

• Ability to work both independently and within a multidisciplinary team
