    Northumberland County Council's Adult Social Care is seeking a compassionate and experienced Adult Social Worker to join our dynamic Community Team. We are committed to providing high-quality support and protection to adults in our community, ensuring their safety, wellbeing, and independence are paramount.

    Your Role:

    As a Community Team member, you will be a vital link in supporting individuals and families facing challenges within their own homes.

    Your Responsibilities will include:

    • Direct Work with Adults: Building strong and trusting relationships with individuals and their families, providing them with the necessary support, guidance, and interventions.
    • Case Management: Taking responsibility for a caseload of individuals, conducting thorough assessments, developing individualized care plans, and ensuring their needs are met.
    • Promoting Wellbeing: Working with individuals and families to address their needs, reduce risk factors, and promote positive outcomes, empowering them to live fulfilling lives.
    • Multi-agency Collaboration: Collaborating effectively with other professionals and agencies involved in an individual's care, including health services, housing providers, and community support organizations.
    • Advocacy and Support: Advocating for the needs and rights of individuals, ensuring they receive the appropriate support and resources.
    • Promoting Positive Change: Encouraging positive change within individuals and their families, empowering them to make choices that support their wellbeing.

    Key Requirements:

    • Social Work Qualification: You must be a qualified Social Worker.
    • Social Care England Registration: Essential for this role.
    • Updated DBS: Essential for working with vulnerable individuals.
    • Experience in Adult Social Care: Demonstrated experience working with adults experiencing difficulties, with a strong understanding of relevant legislation and policy frameworks.
    • Excellent Assessment and Case Management Skills: Proven ability to conduct comprehensive assessments, develop effective care plans, and manage complex cases effectively.
    • Strong Communication and Interpersonal Skills: Ability to build positive relationships with individuals, families, other professionals, and stakeholders.
    • Commitment to Safeguarding Vulnerable Adults: Passion for protecting individuals and ensuring their rights and well-being are paramount.
