Responsibilities- Develop and implement comprehensive care plans tailored to individual needs.
- Conduct thorough and regular assessments, accurately monitoring progress and adapting plans as needed.
- Provide empathetic support and guidance to individuals and their families, fostering a collaborative approach to care.
- Effectively liaise with a wide range of healthcare professionals and agencies to ensure seamless care coordination.
- Maintain meticulous and accurate records and documentation, adhering to all relevant policies and procedures.
- Actively contribute to a positive and supportive team environment, sharing expertise and fostering collaborative working.
- Valid NMC registration
- Proven experience in mental health, ideally within a community setting.
- Excellent communication, interpersonal, and organisational skills are essential.
- Ability to work both independently and as part of a multidisciplinary team.
- A strong commitment to delivering high-quality, person-centred care.
