Pay Rate: NHS Capped Rate
About the RoleJoin our dedicated team at the Pennine Care NHS Trust! We are seeking a compassionate Band 6 CPN Community Psychiatric Nurse to make a significant impact on individuals' lives. Responsibilities- Develop and implement comprehensive care plans tailored to individual needs.
- Conduct thorough and regular assessments, accurately monitoring progress and adapting plans as needed.
- Provide empathetic support and guidance to individuals and their families, fostering a collaborative approach to care.
- Effectively liaise with a wide range of health care professionals and agencies to ensure seamless care coordination.
- Maintain meticulous and accurate records and documentation, adhering to all relevant policies and procedures.
- Actively contribute to a positive and supportive team environment, sharing expertise and fostering collaborative working.
- NMC registration
- DBS on the update service
- Proven experience in mental health, ideally within a Community setting ideally within the NHS
- Excellent communication, interpersonal, and organisational skills are essential
- Ability to work both independently and as part of a multidisciplinary team
- A strong commitment to delivering high-quality, person-centred care
