Job Description:
As a Social Worker in the CWD Team, you will manage a caseload of children with varying disabilities, ensuring that their needs are assessed, and they receive the necessary services to improve their well-being and quality of life.
You will collaborate with families, professionals, and external agencies to develop care plans and ensure children's safety and development.
Responsibilities:
- Carry out assessments to identify the needs of children with disabilities and their families.
- Develop and implement individualised care plans that support the well-being, safety, and development of children.
- Work closely with families, health professionals, and educational teams to coordinate services and interventions.
- Provide ongoing support and advocacy for children with disabilities, ensuring that their voices are heard in decision-making.
- Attend and contribute to multi-agency meetings, including care reviews and safeguarding conferences.
Qualifications:
- Qualified Social Worker registered with Social Work England.
- Experience working with children with disabilities or in a similar statutory social work role.
- Strong knowledge of safeguarding policies, disability legislation, and care planning processes.
