Durham County Council's Children with Disabilities Team is seeking a dedicated and experienced Senior Practitioner to join our team. We are committed to providing high-quality support and advocacy for children with disabilities and their families, empowering them to achieve their full potential.
About the Role and Responsibilities:
- Leading Assessment and Support: Conduct comprehensive assessments of children with disabilities and their families, identifying their needs, strengths, and aspirations. Develop and implement individualised care plans, tailored to the specific needs of each child and family.
- Direct Support and Advocacy: Provide direct support to children and families through one-to-one sessions, group work, and community outreach. Facilitate access to specialist services and resources, such as education, healthcare, and social inclusion opportunities.
- Collaborative Practice: Work collaboratively with other professionals and agencies to ensure children receive the best possible care and support.
- Promoting Inclusion: Advocate for the needs of children with disabilities, ensuring their rights and well-being are protected. Promote inclusion and support children to participate fully in their communities.
- Expertise and Guidance: Offer information, advice, and support to colleagues and other professionals on best practice and relevant legislation related to supporting children with disabilities.
- Maintaining Accurate Records: Maintain accurate records and documentation, ensuring adherence to relevant legislation and guidelines.
- Contribute to Service Development: Contribute to the ongoing development and improvement of the team's service delivery, including participating in quality improvement initiatives.
Key Requirements:
- Qualified Social Worker (DipSW, CQSW, BA Hons in Social Work, or equivalent).
- Social Care England registration.
- Updated DBS check.
- Experience working with children with disabilities and their families.
- Knowledge of relevant legislation and guidance related to supporting children with disabilities.
- Excellent communication, interpersonal, and advocacy skills.
- Ability to build rapport with children, families, and other professionals.
- Commitment to supporting children with disabilities to achieve their full potential and live fulfilling lives.
