Job Description:
We are seeking compassionate and skilled Social Workers to join Cumberland Council's Children with Disabilities Team. The successful candidates will play a crucial role in assessing, planning, and delivering services to children with disabilities and their families, ensuring their well-being and promoting positive outcomes.
Key Responsibilities:
Assessment and Planning:
- Conduct comprehensive assessments of children with disabilities and their families to identify needs and develop tailored support plans.
- Work collaboratively with families, carers, and multi-agency professionals to ensure holistic and effective support.
Service Delivery:
- Provide direct support and interventions to children with disabilities and their families, promoting their well-being and development.
- Advocate for the rights and needs of children with disabilities within the community and across various services.
Case Management:
- Manage a caseload of children with disabilities, maintaining accurate and up-to-date records in line with legal and procedural requirements.
- Monitor and review support plans regularly to ensure they remain effective and responsive to changing needs.
Multi-Agency Collaboration:
- Work closely with education, health, and other social care professionals to coordinate and deliver integrated services.
- Attend and contribute to multi-agency meetings, case conferences, and reviews to ensure cohesive and comprehensive support.
Qualifications:
- Bachelor's degree in Social Work or equivalent qualification.
- Registration with Social Work England (SWE).
- Proven experience working with children with disabilities and their families.
- Strong knowledge of relevant legislation, policies, and best practices in social work.
- Excellent communication, interpersonal, and advocacy skills.
- Ability to manage a complex caseload and work effectively under pressure.
- Commitment to continuous professional development and reflective practice.
