• Conduct thorough assessments to identify needs and support individuals in achieving their goals.
• Advocate for individuals' rights and access to appropriate services and support.
• Promote independence and social inclusion within the community.
• Contribute to a supportive and collaborative team environment dedicated to improving the lives of our clients.

Requirements:

• Registered with Social Care England.
• Minimum 3 years' post-qualification experience as a Social Worker.
• Experience working with individuals with physical disabilities and their families is preferred.
• Excellent communication, interpersonal, and case management skills.
• A driving license and access to a car is desirable, but not essential.
• Strong commitment to person-centered care and ethical practice.
