🔍 Your Mission as Case Manager:
✅ Conduct comprehensive health assessments

✅ Create tailored, multi-disciplinary care plans

✅ Coordinate care across teams - GPs, therapists, social workers & more

✅ Oversee complex care packages for safety, effectiveness & impact

✅ Support with local business development & commissioning

✅ Be a patient advocate and educate families

✅ Monitor outcomes, analyse trends & continuously improve care


🎓 What You'll Need:
✔️ NMC PIN - Registered Nurse in LD or Children's Nursing

✔️ Proven experience in complex care or similar roles

✔️ A proactive, compassionate, organised approach
