Job Title: Complex Care Nurse Case Manager (LD or Children)
Location: Devon-Newton Abbot- Office based
Hours: Full time, Part time, Condensed hours
Salary: £50-55K (Full time) per annum -depending on experience

This Complex Care Provider is looking for highly skilled and experienced Learning Disability and Paediatric nurses to join their expanding team of Nurses, Case Managers and Care assistants. They have a commitment to providing high-quality care to individuals with complex health needs, with an emphasis on patient-centered care, and collaborative approach to healthcare.
About the Role:
We are seeking a compassionate and highly organized Complex Care Nurse Case Manager to join a Complex Care provider in the South West. You will play a pivotal role in coordinating comprehensive care for patients with complex and chronic health conditions, ensuring they receive the right services at the right time in the right place.
Key Responsibilities:
- Conduct thorough assessments of patients health needs, including their medical history, social determinants of health, and goals of care.
- Develop individualized care plans that address patients' complex needs and integrate services from multiple disciplines.
- Coordinate care with healthcare providers, including physicians, specialists, nurses, therapists, and social workers.
- Business development with local Commissioners
- Overseeing packages of care to ensure they are safe and effective
- Advocate for patients' rights and access to appropriate resources, including medications, medical equipment, and community-based services.
- Educate patients and their families about their conditions, treatment options, and self-management strategies.
- Monitor patients' progress and make adjustments to care plans as needed.
- Collaborate with interdisciplinary teams to ensure smooth transitions of care, including hospital discharge planning.
- Analyze data and identify trends to improve patient outcomes and optimize resource utilization.
- Maintain accurate patient records and documentation.
- Participate in team meetings and quality improvement initiatives.
- Stay abreast of the latest clinical guidelines and best practices in complex care management.
Qualifications and Experience:
- NMC PIN
- Nursing qualification in Learning disabilities or/and Children Nursing
