Stockton Borough Council's First Contact Team is seeking a dedicated and experienced Adult Social Worker to join our team. We are committed to providing high-quality support and assessments to adults in need, ensuring they receive the right help at the right time.
About the Role and Responsibilities:
... - Providing First Point of Contact: Receive and assess referrals for adults requiring social care support, acting as the first point of contact for individuals and their families.
- Conducting Comprehensive Assessments: Conduct comprehensive assessments to identify the individual's needs, strengths, and risks, using a person-centered approach.
- Developing Individualised Care Plans: Develop and implement individualised care plans, including interventions and support strategies, to meet the individual's needs and promote their well-being.
- Collaborative Practice: Work collaboratively with individuals, families, and other professionals, including health services, housing providers, and the police, to deliver timely and effective support.
- Providing Information and Advice: Provide information, advice, and guidance to individuals and families on a range of social care services and benefits available.
- Advocating for Individuals: Advocate for the needs of individuals, ensuring their rights and well-being are protected.
- Maintaining Accurate Records: Maintain accurate records and documentation, ensuring adherence to relevant legislation and guidelines.
- Contributing to Team Development: Contribute to the ongoing development and improvement of the team's service delivery.
Key Requirements:
- Qualified Social Worker (DipSW, CQSW, BA Hons in Social Work, or equivalent).
- Social Care England registration.
- Updated DBS check.
- Experience working within Adult Social Care, particularly in a first contact or assessment role.
- Excellent communication and interpersonal skills.
- Strong analytical and problem-solving abilities.
- Ability to work effectively under pressure and manage competing demands.
- Commitment to person-centered practice and a commitment to improving the lives of vulnerable adults.
