Key Responsibilities:
Conduct assessments and develop care plans to support children with disabilities and their families.
Work closely with health, education, and other partner agencies to provide a multi-agency approach to care.
Ensure all interventions comply with relevant legislation, policies, and procedures.
Provide safeguarding and risk management support for children with disabilities.
Advocate for children and young people, ensuring their voices are heard and their rights are upheld.
Participate in team meetings, case conferences, and training opportunities.
Monitor and review cases to ensure that interventions remain appropriate and effective.
Essential Requirements:
Qualified social worker (Degree in Social Work or equivalent) with current Social Care Wales registration.
Experience working within children services, ideally within a disabilities team.
