About the Role:

- Provide specialist social work support to adults with learning disabilities and autism, promoting their independence, well-being, and inclusion within the community.
- Conduct comprehensive needs assessments and develop person-centered care plans tailored to the unique needs of each individual.
- Collaborate effectively with individuals, their families, carers, and other professionals to achieve positive outcomes.
- Advocate for individuals' rights and access to appropriate services and support.
- Contribute to a supportive and collaborative team environment focused on best practice and positive change.
Requirements:
- Registered with Social Care England.
- Minimum 3 years' post-qualification experience as a Social Worker.
- Significant experience working with adults with learning disabilities and/or autism is essential.
- Excellent communication, interpersonal, and case management skills. Experience with challenging behaviour is highly desirable.
- Commitment to person-centered care and ethical practice.
