Are you passionate about making a positive impact on the lives of young adults transitioning from care to independence? Join our dynamic team as a Leaving Care Personal Advisor and play a crucial role in empowering and supporting young individuals on their journey to a brighter future!
As a Leaving Care Personal Advisor, you will be responsible for building meaningful relationships with young people, aged 16-25, who are leaving the care system. Your role will involve:
Conducting comprehensive assessments to identify individual needs and strengths.
Developing personalised care plans to address educational, employment, housing, and emotional well-being goals.
Providing practical advice and support to enhance life skills, including budgeting, cooking, and self-care.
Advocating on behalf of young adults to ensure access to essential services and resources.
Collaborating with external agencies and professionals to create a holistic support network.
Qualifications :
- Relevant experience in social work.
- Strong communication and interpersonal skills.
- Understanding of the challenges faced by young people leaving care.
- Ability to work independently and as part of a collaborative team.
- Commitment to continuous professional development.
