Responsibilities:
In this role, you will work with individuals in their homes and local community settings, assessing their needs, creating tailored care plans, and delivering therapeutic interventions. You will play a key role in improving the quality of life for adults with physical, mental health, and cognitive challenges, helping them regain or maintain their independence.
Key Responsibilities:
- Conduct comprehensive assessments of individuals in their home and community environments.
- Develop and implement person-centred rehabilitation and intervention plans.
- Provide advice and support on daily living activities, adaptive equipment, and home modifications.
- Work closely with families, carers, and multidisciplinary teams to ensure holistic care and support.
- Monitor and evaluate the progress of service users, adjusting interventions as required.
- Offer emotional support and guidance, helping individuals adjust to physical or cognitive changes.
- Maintain accurate records and reports in line with relevant health and social care standards.
Qualifications and registrations:
- Qualified Occupational Therapist
- HCPC registered
