Job Purpose:
We are looking for an experienced Social Worker to join the Adults Older Persons Team in Powys on an agency basis. In this role, you will provide vital support and care planning for older adults, working closely with service users, their families, and multi-disciplinary teams to ensure person-centered care that promotes dignity, independence, and well-being. The position offers flexibility with competitive agency rates, making it an excellent opportunity for professionals seeking temporary or contract work.
Key Responsibilities:
- Assessment and Support Planning: Conduct assessments of older adults' needs, including physical, social, and emotional well-being, and develop care and support plans tailored to individual needs.
- Care Coordination: Arrange and coordinate community services, residential care, and other appropriate interventions to ensure older persons receive the necessary support for independent living or care within the appropriate setting.
- Safeguarding and Risk Management: Identify safeguarding concerns and implement appropriate measures in line with local authority safeguarding policies, ensuring the protection and safety of vulnerable older adults.
- Mental Capacity Assessments: Conduct mental capacity assessments and work within the framework of the Mental Capacity Act to support older adults in making informed decisions about their care.
- Multi-Agency Working: Collaborate with health professionals, housing services, and third-sector organizations to ensure a comprehensive and integrated approach to care for older adults.
- Crisis Intervention: Provide immediate support in crisis situations, ensuring appropriate and timely interventions are in place to support the safety and well-being of older persons.
- Advocacy and Empowerment: Advocate on behalf of service users to ensure their rights are upheld and that they have access to the services they need, promoting independence and well-being.
- Record Keeping: Maintain accurate and up-to-date case records, ensuring all documentation is completed in line with local authority policies and legal requirements.
- Legal Framework Compliance: Apply relevant legislation, such as the Care Act, Mental Capacity Act, and Deprivation of Liberty Safeguards, in the planning and delivery of care.

Qualifications:
- A degree in Social Work (or equivalent).
- Current registration with Social Care Wales
