Abbott manufacture specialist medical food and equipment for internally (through tubes) fed patients, we aim to improve patient quality of life and meet their nutritional needs.
Hospital2Home's (H2H) role is to support the patients feeding once they have left the hospital and returned home. Hospital2Home's (H2H) new patient role is integral to H2H. As the new patient coordinator, you will be responsible for making the first contact with the patient or carer once their healthcare professional registers them to H2H. You will set up their account, confirm all the patient and carers detail's, arrange their first order and then check in with the patient or carer once the order has been delivered. You might need to liaise with their healthcare professional or GP surgery to ensure a smooth transition to our service for the patient. This can be an extremely challenging and daunting time for our patients and so we must be friendly, helpful and most importantly, empathetic.
H2H work closely with our dedicated nursing service who oversee the clinical aspects of patient feeding. Our H2H customer service coordinators play an integral frontline role and are committed to delivering the highest levels of service to our patients. This is role for someone who wants to make a difference to people's lives. All new starters will undertake a 2-month training program overseen by our team trainer.
The working hours will be Monday-Friday 9am-5:30pm office based in Maidenhead or Sittingbourne.
