Newcastle City Council is seeking a dedicated and experienced Adult Social Worker to join our Prevention Service. We are committed to supporting adults to live fulfilling and independent lives, preventing unnecessary care and promoting well-being.
About the Role:
You will be part of a team dedicated to:
- Providing early intervention and support to adults at risk of needing formal care services.
- Conducting assessments, developing individualised care plans, and delivering support to adults to address their needs and reduce risks.
- Working collaboratively with individuals, families, and other agencies to promote independence and prevent unnecessary care.
- Maintaining accurate records and documentation, adhering to relevant legislation and guidelines.
- Participating in regular supervision and professional development opportunities.
Responsibilities:
- Conduct comprehensive assessments of adults referred to the Prevention Service, identifying their needs, strengths, and risks.
- Develop and implement individualised care plans, including interventions and support strategies.
- Provide direct support to individuals, including advocacy, information, and advice.
- Work collaboratively with individuals, families, and other professionals, including health services, housing providers, and voluntary sector organizations, to deliver timely and effective support.
- Facilitate access to resources and services to promote independence and well-being.
- Maintain accurate records and documentation, ensuring adherence to relevant legislation and guidelines.
- Contribute to the ongoing development and improvement of the team's service delivery.
Key Requirements:
- Qualified Social Worker (DipSW, CQSW, BA Hons in Social Work, or equivalent).
- Social Care England registration.
- Updated DBS check.
- Experience working within Adult Social Care, particularly in a prevention or early intervention role.
- Excellent communication and interpersonal skills.
- Strong analytical and problem-solving abilities.
- Ability to work effectively under pressure and manage competing demands.
- Commitment to person-centered practice and a commitment to promoting independence and well-being.
