Responsibilities include:
- Conduct comprehensive assessments to identify the needs of clients with learning disabilities and develop appropriate care plans.
- Carry out mental capacity assessments in accordance with the Mental Capacity Act 2005, and make Deprivation of Liberty Safeguard (DoLS) applications as required to ensure legal compliance and protect the rights of individuals with learning disabilities.
- Provide immediate support and intervention in response to crises or emergencies involving individuals with learning disabilities.
- Work closely with a multidisciplinary team, including psychologists, speech and language therapists, nurses, and other healthcare professionals, to deliver coordinated and tailored care.
- Maintain accurate and timely records of assessments, interventions, and care plans, ensuring all documentation aligns with council policies and legal standards specific to learning disabilities.
Requirements:
- Relevant social work qualification
- 2 experience in adult social work, including assessment and care planning.
