You will play a vital role in supporting adults facing mental health challenges. Your duties will include carrying out comprehensive assessments to understand individual needs, creating tailored care plans, and working with a range of services to ensure timely access to the right mental health support.

Responsibilities include:
- Conduct comprehensive assessments to identify clients' mental health needs and develop appropriate care strategies.
- Perform mental capacity assessments in line with the Mental Capacity Act 2005, and make Deprivation of Liberty Safeguard (DoLS) applications as needed to ensure legal compliance and safeguard individuals' rights.
- Provide immediate support and intervention in response to mental health emergencies and crises.
- Collaborate with a multidisciplinary team, including psychiatrists, psychologists, nurses, and other healthcare professionals, to deliver coordinated, holistic care.
- Maintain accurate and timely records of assessments, interventions, and care plans, ensuring all documentation meets council policies and legal requirements.
Requirements:
- Relevant social work qualification
- 2 experience in adult social work, including assessment and care planning.
