As a Social Worker in the Adult Generic Team, you will play a pivotal role in assessing the needs of adults with varying levels of vulnerability and supporting them to maintain independence and well-being. This role involves conducting person-cantered assessments, planning care, and coordinating services to address a broad range of needs, from physical health and mental well-being to social support. You will work closely with adults, their families, and other professionals to ensure comprehensive care and support within the community.
Responsibilities include:
... - Conduct holistic assessments of adults under the Care Act 2014, covering physical, mental, and social needs to develop personalised care plans.
- Develop and implement care plans that promote independence, enhance well-being, and address specific needs in areas such as housing, health, and social care.
- Provide early intervention and prevention services to reduce risks of deterioration or crisis, supporting adults to remain in their homes and communities.
- Collaborate with multidisciplinary teams including health professionals, housing services, and community organisation's ensure integrated and coordinated support.
- Identify safeguarding concerns and implement appropriate actions to protect adults from abuse, neglect, or harm.
- Review care plans regularly to adapt to changing needs and circumstances, ensuring ongoing relevance and effectiveness.
- Advocate for adults' rights, ensuring they access the necessary resources, benefits, and services to lead fulfilling lives.
- Ensure compliance with statutory duties, including safeguarding procedures, while adhering to best practice standards.
Requirements:
- 2 years experience post qualification
