Randstad are currently recruiting for a Qualified Social Worker to Lancashire County Council's Adult CHC team located in Preston. This is a part time role, offering up to £41 per hour, DOE.

Responsibilities:
- Undertake care act assessments for CHC cases and attend CHC DST's and CHC reviews.
- Must have substantive experience of CHC cases.
- Complete care act assessments for individuals with complex nursing needs, completing care plans and reviewing cases.
- Good working knowledge of the National Framework for NHS Continuing Healthcare and NHS funded Nursing Care July 2022 (Revised) as well as The Care Act (2014).
- Good assessment skills and an ability to identify what care needs fall within the responsibility of the Local Authority will be required for this role.
- You will work within an MDT approach, undertaking Social Care Assessments and undertaking Decision Support Tools for individuals the NHS and Local Authority have identified as requiring a review of CHC Eligibility. Social Workers will be working from home for the majority of the time, however visits to individuals will be throughout Lancashire.
Requirements:
- Social Work Degree
- Enhanced DBS
- Social Work England registered
- It is essential that you have experience working with Liquid Logic as the Project does not afford time for training beforehand.
