Responsibilities of a Social Worker within the Child Health and Disability Team:
- The team provides services to children and young people who have severe or significant learning disability, physical disability, sensory impairment or profound communication impairment.
- Specialise in supporting children who need continuing care
- Work closely with health, education and other various external agencies
- Respond to new referrals to the team and ensure you provide an effective initial response and develop long term relationships with the children, young people, families and carers
Requirements:
- Social Work Qualification
- 1+ years experience
- Newly Qualified Social Workers currently working through their ASYE are welcome to apply
- Social Work Wales registration or eligible to apply
- A passion for achieving the best outcomes for children through direct work
- A creative and innovative approach
