Overview:
- Conduct comprehensive assessments for children with disabilities and their families.
- Develop and review care plans, ensuring they meet individual needs and improve the quality of life for children and their families.
- Provide advice, support, and guidance to families, schools, and other professionals.
- Liaise with partner agencies to coordinate multi-agency interventions and services.
- Work collaboratively with children, parents, and carers to ensure that their voices are heard in all aspects of care planning.
- Safeguard children and young people, taking necessary actions when concerns arise.
- Advocate for children's rights and ensure their needs are prioritized in decision-making.
Requirements:
- Social Work England registered
- Social Work Degree or equivalent
- Enhanced DBS

- Children's Social worker
