Overview:
- Assessment and Case Management: Undertake assessments of children and young people with disabilities and their families, ensuring a thorough understanding of their needs and identifying the most effective interventions.
- Care Planning: Develop and review tailored care plans that promote the well-being and independence of children and young people, ensuring they have access to the necessary services and support.
- Safeguarding and Advocacy: Safeguard and promote the welfare of children with disabilities, advocating for their rights and ensuring their voices are heard in decision-making processes.
- Multi-Agency Collaboration: Work closely with health, education, and other relevant professionals to ensure coordinated support for children and families, including attendance at multidisciplinary meetings and coordinating services.
- Parent and Family Support: Provide emotional and practical support to families, ensuring that they are empowered to manage challenges and access the resources they need.
- Record-Keeping and Reporting: Maintain accurate and up-to-date case records, prepare reports for reviews and meetings, and ensure that all actions are documented in accordance with council policies and legal frameworks.
Requirements:
- Social Work England registered
- Social Work Degree or equivalent
- Enhanced DBS
Experience:
- Children's Social worker
