If so, Randstad are recruiting for an Adults Senior Social Worker to join Leeds Council - Hospital Discharge Team. This is a full-time role offering Up to £35/hr

Overview:
- We are looking for 3 social workers with experience of working in Adult Social Care, hospital discharge neighbourhood teams.
- The social worker should be experienced with good understanding of the social worker's processes including assessment, intervention and review and the key legislation that underpin this process.
- They should have the ability to carry a caseload of complex cases and undertake assessment of service users and carer needs using a person-centred approach in accordance with agreed statutory and directorate procedures.
- Able to assess capacity and coordinate best interest meetings and decision making for those who lack capacity to make decision for their discharge destination.
- Able to assess and manage risks and ensure any safeguarding is dealt with in timely manner.
- To have a good understanding of the strengths-based approach and how to apply this in practice.
- These are AP level positions pay rate £35 per hour umbrella
- Must have previous experience of working in a hospital team
- 3 months initial placement
Requirements:
- Social Work England registered
- Social Work Degree or equivalent
- Enhanced DBS

- Adult's Social worker
