Responsibilities:
- The team focus on the Home First approach to discharge. You would be supporting with triaging referrals with the Homefirst and Reablement Team.
- Undertaking a duty role and working with people throughout their journey from hospital to rehabilitation
To be considered for the role, ideally you will have:
- Minimum 2 years' experience in a similar social work setting.
- Diploma/Degree in Social Work.
- Registered with Social Work England.
