Responsibilities of a Social Worker in the Hospital Discharge Team:
- To arrange and undertake Care Act Assessments including home visits, as per the Discharge to Assess guidance.
- Arrange and undertake Mental Capacity Assessments and Best Interest Meetings
- Implement packages of care as per Discharge to Assess guidelines.
- Work in partnership with health and community colleagues.
Requirements:
- Social Work Qualification
- 4-5 years post qualifying experience
- Social Work England registration
- Practice Educator training would be desirable
- A creative and innovative approach
- Experience of working in a similar team would be desirable
