Randstad are recruiting for a Qualified Social Worker to join Bolton Council's Intermediate Care team. This is a full time role offering a fantastic rate of £30 per hour.

Responsibilities:
- You will be an excellent communicator who will put the person at the forefront of any intervention, valuing their uniqueness, individuality, and strengths.
- You will be the professional that enables members of our community to live the best lives possible supporting them to have choice and control ensuring their voice is heard.
- You will be committed to delivering best practice in social work, promoting professional values and standards.
- You will be inspired to grow as a member of the social work profession.
- You will work alongside an experienced MDT workforce to support the people of Bury to return to or remain in their own home after a period of rehabilitation.
- You will work within a small social care team to support the home-based and based-based areas of intermediate care.
- You will use a strengths-based approach and always thinking 'tech-first' will support the Bury vision and values.
- You will create long term plans (36 IMC Beds - therapy led rehab for 6 week period).
- You will make complex discussions with families - discharge planning meetings inducing 24 hour care etc.
Requirements:
- Social Work England registered
- Social Work Degree or equivalent
- Enhanced DBS
