Randstad are recruiting for a Qualified Social Worker to join Bolton Council's Intermediate Tier Services. This is a full time role offering a fantastic rate of £30 per hour.

Responsibilities:
- This service covers Admission Avoidance; Intermediate Care and Discharge to Assess bed-based units.
- You will have an understanding of the needs and aspirations of individuals and supporting them to enhance their quality of life and promote independence.
- You will work in one of the designated services and undertake assessments/reviews of need under the Care Act 2014.
- Assist with support planning as necessary and also link people into universal and voluntary services.
- You will also assist colleagues within other areas of the services as necessary and support with complex assessments, mental capacity and best interest assessments.
Requirements:
- Social Work England registered
- Social Work Degree or equivalent
- Enhanced DBS

- Adults
