If so, Randstad are recruiting for a Social Worker to join Halton Council's Hospital Team. This is a full-time role offering upto £32 per hour.

Responsibilities:
- Assessment and Support: Conduct comprehensive social assessments for adult patients, identifying their needs and determining appropriate interventions.
- Care Planning: Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Advocacy: Act as an advocate for patients, ensuring their views and wishes are heard and considered in the care process.
- Resource Coordination: Identify and facilitate access to community resources, services, and support networks to assist patients and their families.
- Crisis Intervention: Provide immediate support and solutions in crises, ensuring the safety and well-being of patients.
- Documentation: Maintain accurate and up-to-date case records, ensuring all documentation meets legal and professional standards.
- Team Collaboration: Work effectively within a multidisciplinary team, sharing information and expertise to achieve the best outcomes for patients.
Requirements:
- Social Work England registered
- Social Work Degree or equivalent
- Enhanced DBS
