Responsibilities:
- We have 2 Pathways Teams.
- The Pathways Teams help our Cared For and Care Experienced young people as they move towards independence.
- At around 15 years and 9 months of age young people will move across to Pathways Team 1.
- At this juncture they will still have an Independent Reviewing Officer (till 18years) who will regularly review their care plan.
- As young people move closer to adulthood, they will have been introduced to a new worker from the Pathways 2 Team.
- This worker will get to know them and become their new worker as they reach 18. The Pathways teams support young people towards living independently. Y
- oung people will have a Pathway Plan that looks at their health, education, employment and training, support, relationships, and things that are important to the young person.
- Pathways workers will complete direct work with young people to help them understand their experiences and also has engagement groups where our children and young people give feedback on their experiences to help us make sure our services work for them.
Requirements:
- Social Work Qualification or equivalent
- Social Work England registered
- Enhanced DBS
