Key Responsibilities:
- Assessment: Conduct detailed assessments of children's and families' needs to identify risks and areas for support.
- Intervention Planning: Develop and implement tailored intervention plans based on assessment findings to support positive outcomes.
- Case Management: Manage a caseload effectively, ensuring timely and thorough responses to the needs of children and families.
- Collaboration: Work in partnership with other social workers, healthcare professionals, educational staff, and community agencies.
- Support & Advocacy: Provide emotional and practical support to families, advocating for their needs and ensuring their voices are heard.
- Documentation: Maintain accurate and up-to-date case records and documentation by organizational and regulatory standards.
Requirements:
- Social Work England registered
- Social Work Degree or equivalent
- Enhanced DBS
