About the Role:

We are looking for an experienced and passionate Social Worker to join Wrexham's Family Support Team on an agency basis. This role involves working directly with children and families to provide early intervention, support, and safeguarding to improve outcomes and prevent family breakdowns. The successful candidate will be skilled in assessments, intervention planning, and multi-agency working.

Key Responsibilities:

• Manage a caseload of children and families requiring support and intervention.
• Undertake assessments, including Child and Family Assessments, to identify needs and risks.
• Develop and implement support plans in partnership with families, ensuring the child's welfare is at the center of all decision-making.
• Work closely with other professionals, including schools, health services, and partner agencies, to coordinate support.
• Provide direct work and interventions to children and parents to strengthen family relationships.
• Monitor and review support plans, ensuring timely progress and positive outcomes.
• Attend multi-agency meetings, including Child in Need (CIN) and Child Protection (CP) conferences.
• Ensure all work complies with relevant legislation, policies, and procedures.

Requirements:

• Degree in Social Work
• Social Work Wales Registration
