Responsibilities:
- Conduct prompt, person-centered assessments for adults referred to the Duty Team.
- Work collaboratively with multi-disciplinary teams, including health professionals, housing services, and voluntary organisations.
- Develop and implement care and support plans tailored to individual needs.
- Manage crisis interventions, ensuring the safety and well-being of service users.
- Safeguard adults at risk by identifying and addressing concerns, adhering to national and local safeguarding protocols.
- Maintain accurate, up-to-date case records and reports in compliance with legal and organisational standards.
- Provide advice and guidance to individuals, families, and carers.
- Contribute to service development and promote best practices within the team
Qualifications and registrations:
- Qualified Social Worker
- SSSC registered in the Social Work part of the register
