Randstad is seeking a Qualified Social Worker to work within Lancashire County Council's Hospital Discharge Team. This full-time position offers a competitive pay rate of £37 per hour.

Responsibilities:
- Facilitate the smooth and safe discharge of residents to suitable settings.
- Implement various Discharge to Assess and re-ablement pathways to promote independence outside of the hospital environment.
- Conduct assessments to identify immediate and long-term care needs, supporting individuals in achieving their care goals.
- Collaborate with a multi-agency team to ensure effective, safe, and timely discharges across different hospital sites.
- Conduct Mental Capacity assessments, participate in Best Interest Meetings, and attend MDT meetings as required.
- Contribute to a Duty rota.
Requirements:
- Registration with Social Work England
- Enhanced DBS clearance
- Social Work Degree or equivalent qualification
- Experience working with adults
