About the Role
We are seeking a dedicated and experienced Social Worker to join our Intermediate Care Services team. This role focuses on improving outcomes for adults with care and support needs, as well as their carers, through strengths-based assessments, planning, and interventions.
As part of this team, you will contribute to safeguarding adults at risk, ensuring your practice aligns with the Care Act 2014, statutory guidance, and professional standards.
Key Responsibilities
- Undertake strengths-based assessments and planning to identify and address the needs of adults with care and support requirements and their carers.
- Deliver direct work and interventions in accordance with Care Act 2014 guidelines, policies, and procedures.
- Conduct safeguarding duties to protect adults at risk of abuse or neglect.
- Work transparently, ensuring accountability and adhering to professional ethics, values, and standards.
- Collaborate with other professionals and stakeholders to deliver holistic care and support solutions.
Requirements
- Qualified and registered Social Worker with a solid understanding of the Care Act 2014.
- Experience in strengths-based practice and working with adults with care and support needs.
- Strong knowledge of safeguarding principles and ability to deliver interventions to protect vulnerable adults.
- Commitment to professional accountability, transparency, and ethical practice.
