As a Social Worker in the LD & Autism Team, you will:
Support adults with learning disabilities and autism, ensuring they receive the right care and support.
Conduct assessments and care planning in line with the Care Act 2014.
Work collaboratively with health professionals, families, and community services.
Promote independence, choice, and person-centered care.
Provide safeguarding interventions where necessary.
Advocate for service users, ensuring their voices are heard and their rights upheld.

What You Need:
A minimum of 3 years' experience as a qualified Social Worker.
Registration with Social Care England.
Experience working with adults with learning disabilities and autism.
Knowledge of The Care Act 2014, Mental Capacity Act, and Adult Safeguarding policies.
Strong assessment, risk management, and casework skills.
