Job Summary:
The Localities Social Worker provides a range of support to individuals, families, and communities within a designated locality. This role involves assessing client needs, developing care plans, and providing support and advocacy to ensure clients receive appropriate services. The social worker collaborates with various community resources and agencies to enhance client well-being and foster self-sufficiency.
Key Responsibilities:
Client Assessment and Case Management:
- Conduct comprehensive assessments of client's needs, strengths, and support systems.
- Develop individualised care plans tailored to client's specific circumstances.
- Monitor and evaluate client progress, adjusting care plans as necessary.
- Maintain accurate and up-to-date case records in accordance with department policies.
Support and Advocacy:
- Provide counselling and emotional support to clients and families.
- Advocate for client's access to services and resources, including health care, housing, employment, and education.
- Assist clients in navigating social service systems and obtaining necessary benefits.
Community Engagement and Collaboration:
- Build and maintain relationships with community organisations, service providers, and other stakeholders.
- Participate in community outreach and education activities to promote awareness of social services and resources.
- Coordinate with other professionals and agencies to ensure a holistic approach to client care.
Crisis Intervention:
- Respond to and manage emergency situations involving clients, such as homelessness, domestic violence, or mental health crises.
- Provide immediate support and develop crisis intervention plans to ensure client safety and stability.
Program Development and Evaluation:
- Contribute to the development and implementation of social service programs and initiatives within the locality.
- Evaluate program effectiveness and provide recommendations for improvement.
Compliance and Professional Development:
- Ensure compliance with all relevant laws, regulations, and policies governing social work practice.
- Participate in ongoing training and professional development to stay current with best practices and emerging trends in social work.
