• Case Management: Manage a caseload of young people leaving care, providing ongoing support and guidance as they navigate key life transitions.
• Develop and Implement Care Plans: Work collaboratively with young people, families, and other professionals to develop and implement personalised care plans that address their individual needs, aspirations, and risks.
• Provide Direct Support: Offer practical and emotional support to young people in their transition into adulthood, including assistance with housing, education, employment, relationships, and other areas of their lives.
• Advocate for Young People's Needs: Champion the best interests of young people, advocating for their rights and ensuring they have access to appropriate services and resources.
• Collaborate with Multi-Agency Partners: Work closely with other professionals, including housing providers, education providers, health services, and other agencies, to ensure a coordinated approach to support.
• Contribute to Service Development: Engage in ongoing professional development and actively participate in initiatives to improve the quality of services for young people leaving care.
• Attend Reviews: Participate in reviews of care plans, ensuring they are meeting the needs of young people and promoting positive outcomes.
• Maintain Accurate Records: Maintain clear and accurate records of all work undertaken, adhering to relevant policies and procedures.

Essential Requirements:

• Qualified Social Worker: Must hold a relevant Social Work qualification (e.g. DipSW, BSc, MA) and be registered with Social Care England.
• Experience with Children in Care: Proven experience working with children in care, ideally with a focus on supporting transitions into adulthood.
• Strong Assessment and Planning Skills: Ability to conduct comprehensive assessments, identify needs, and develop effective care plans.
• Excellent Communication and Interpersonal Skills: Ability to build rapport with young people, families, and other professionals, fostering effective communication and collaboration.
• Commitment to Child-Centred Practice: A passion for safeguarding children and a dedication to ensuring their safety and well-being.
