Responsibilities:
- Conduct comprehensive social work assessments to identify patients' needs and develop personalised discharge plans.
- Collaborate effectively with multidisciplinary teams, including medical staff, nurses, therapists, and community services.
- Arrange appropriate care packages, including home care, equipment, and financial support.
- Provide information, advice, and advocacy to patients and their families.
- Safeguard vulnerable adults and children, adhering to relevant legislation and policies.
- Maintain accurate records and contribute to service development.
- Work to prevent delayed discharges and promote patient independence.
Qualifications and registrations:
- Qualified Social Worker
- SSSC registered as a Social Worker
Skills:
- Car driver
- At least 3 years hospital discharge experience
