Responsibilities:
As an Adult Social Worker in a Locality Team, you will:
- Provide person-centered support: Assess the needs of individuals and their families, creating care plans tailored to their circumstances.
- Advocate for clients: Work collaboratively with individuals, carers, and other professionals to promote independence, dignity, and wellbeing.
- Safeguard vulnerable adults: Respond to safeguarding concerns with professionalism and care, ensuring that those at risk are protected.
- Work collaboratively: Build strong relationships with partners across health, housing, and community services to deliver seamless support.
- Promote resilience: Empower clients by fostering independence and supporting them to access community resources.
Qualifications and registrations:
- Qualified Social Worker
- SSSC registered in the Social Work part of the register
