Job Title: Adult Social Worker - Localities Team
Salary: £31,372 to £41,438 (depending on experience)

Position Overview: We are seeking a passionate and experienced Adult Social Worker to join our dynamic team. The successful candidate will be responsible for conducting assessments, developing care plans, and coordinating support services to meet the needs of adults within a designated geographic area.
Key Responsibilities:
- Conduct comprehensive assessments of adults' needs, strengths, and preferences, using a person-centered approach.
- Develop and implement care plans in collaboration with adults, their families, and relevant stakeholders, ensuring that interventions are tailored to meet individual needs and promote independence.
- Provide information, advice, and advocacy to adults regarding their rights, entitlements, and available support services.
- Coordinate and facilitate access to community resources, including health and social care services, housing support, employment opportunities, and leisure activities.
- Work collaboratively with multi-disciplinary teams, including health professionals, care providers, housing agencies, and community organizations, to ensure coordinated and integrated support for adults.
- Monitor and review the effectiveness of support plans, making adjustments as necessary to ensure that goals are achieved and outcomes are maximized.
- Maintain accurate and up-to-date records in accordance with organizational policies and statutory requirements.
Qualifications and Experience:
- Degree in Social Work or equivalent qualification.
- Registered with Social Work England.
- Previous experience working as a social worker in adult social services.
- Sound knowledge of adult social care legislation, policies, and procedures.
- Excellent assessment, communication, and interpersonal skills.
- Ability to work effectively under pressure and manage a diverse caseload.
- Commitment to promoting independence, dignity, and social inclusion among adults.
